Release of Liability

I release Golden Healer Service Dogs organization or person(s) from all liability and responsibility.

Assumption of Risk

I, the undersigned participant, understand that participation in Puppy Yoga involves physical activity and interaction with animals. I acknowledge that: • Yoga involves physical exertion and may carry risk of injury. • Interaction with puppies/dogs may include behaviors such as jumping, scratching, nipping, or accidental contact. • There is a possibility of allergic reactions, slips, falls, or other unforeseen risks. I voluntarily assume all risks associated with participation in this event.

Health & Fitness Confirmation

I certify that: • I am physically able to participate in yoga activities. • I have no medical condition that would prevent safe participation, or I have consulted a medical professional. • I will participate at my own pace and notify staff if I feel discomfort or need to stop.

Puppy Interaction Agreement

I agree to: • Treat all puppies/dogs with kindness, respect, and care. • Follow all instructions provided by GHSD staff and handlers. • Not engage in rough handling, feeding (unless approved), or unsafe behavior. • Immediately report any incidents or concerns to staff. I understand that failure to follow these guidelines may result in removal from the event without refund.

Photo & Video Release (Marketing Consent)

I understand that this event will be photographed and video recorded. By signing below, I: • Grant permission to Golden Healer Service Dogs (GHSD) to use my image, likeness, voice, and participation in: o Marketing materials o Social media o Website content o Promotional videos o Fundraising campaigns • Understand that these materials may be used without compensation. • Waive any right to inspect or approve final media.

Indemnification

I waive my right to sue this organization or person(s) for any injuries I may sustain. In consideration of being allowed to participate, I hereby release, waive, and discharge: Golden Healer Service Dogs (GHSD), its directors, officers, volunteers, instructors, sponsors, and affiliates from any and all liability, claims, demands, or causes of action arising out of or related to: • Personal injury • Property damage • Illness or allergic reactions • Animal-related incidents whether caused by negligence or otherwise, to the fullest extent permitted by law.
